The Healthy Start home-visiting program employs the Community Health Worker model to offer educational and practical support to high-risk pregnant and parenting women with infants aged 0-18 months. This program helps them navigate the challenges of pregnancy and early parenthood. 

Through the Seed-And-Scale Match Fund, Community Health Workers will conduct monthly home visits, providing trauma-informed care and guidance through the evidence-based Parents as Teachers model. Services include perinatal health education, diabetes and heart disease prevention, health assessments and referrals, and socio-emotional support. The program also features screenings for maternal depression and Adverse Childhood Experiences, and developmental screenings for infants. Additionally, the Family Benefits Program helps eligible community members access state-subsidized health insurance and food benefits to reduce financial strain and address food insecurity. 

MCHC staff, who are embedded in the local community and fluent in English and Spanish, serve diverse populations across Chester County and Montgomery County, PA. 

In addition to providing direct services, MCHC is actively working on systemic changes to support children and caregivers, focusing on reducing racial disparities in health outcomes. Initiatives such as the Perinatal Periods of Risk project aim to assess these disparities and implement strategies to enhance trust, mental well-being, diversity in healthcare, and service integration. Staff use the Apricot case management database to track participant demographics, progress, and outcomes, enabling real-time data collection and monitoring of program goals.
